Novo Nordisk reported DKK 148,551 million in adjusted sales for the first half of 2026, up 2% at constant exchange rates, while its share of the combined Novo Nordisk and Eli Lilly incretin revenue pool fell to 39.1% from 53.6% a year earlier. Obesity care grew 19%. The company’s own revenue barely moved.

Quick Answer

Novo Nordisk’s adjusted H1 2026 sales were DKK 148,551 million (roughly USD 23.2 billion at the company’s disclosed average rate of 6.40 DKK per USD), with adjusted operating profit of DKK 66,247 million and an adjusted operating margin of 44.6%. According to Axis Intelligence Research, the Incretin Revenue Balance (IRB) – Novo Nordisk’s share of the two-company incretin revenue pool it shares with Eli Lilly – stands at 39.1% as of June 30, 2026, down 14.5 percentage points year over year. Semaglutide-branded products generated 75.5% of adjusted sales.

Key Findings

  1. According to Axis Intelligence Research, Eli Lilly captured 94.8% of the growth in the combined Novo Nordisk and Eli Lilly incretin revenue pool between H1 2025 and H1 2026, taking USD 12,959 million of the USD 13,664 million increment.
  2. Novo Nordisk’s adjusted sales grew 2% at constant exchange rates to DKK 148,551 million in H1 2026, per the company’s financial report filed with the SEC on August 4, 2026.
  3. According to Axis Intelligence Research, the Semaglutide Dependence Ratio (SDR) reached 75.5% in H1 2026, up from 72.8% a year earlier, meaning three-quarters of Novo Nordisk revenue now traces to a single molecule.
  4. Wegovy pill generated DKK 5,474 million in its first two quarters and passed 5 million cumulative US prescriptions by the week ending July 17, 2026, which Novo Nordisk describes as the strongest GLP-1 volume launch recorded in the United States.
  5. Reported H1 2026 sales of DKK 175,311 million were 15.3% higher than adjusted sales because of a one-off DKK 26,760 million reversal of a 340B rebate provision, an accounting event with no cash impact.

How Much Revenue Does Novo Nordisk Make in 2026?

Two numbers describe the same six months, and the gap between them is the story of Novo Nordisk’s 2026 reporting.

Reported net sales for H1 2026 were DKK 175,311 million, up 13% in Danish kroner. Adjusted net sales were DKK 148,551 million, up 2% at constant exchange rates. The difference is a DKK 26,760 million reversal, booked in Q1, of a rebate provision the company had carried against the US 340B Drug Pricing Program, administered by the Health Resources and Services Administration. The reversal has no cash impact. It inflates reported sales by 15.3%, and Novo Nordisk introduced an entire non-IFRS reporting framework in 2026 specifically to strip it out.

Mia Scarlett: Reported operating profit rose 27% at constant rates. Strip the provision reversal and the impairment, and the operating business grew 2% – which is also roughly what its sales grew. A quarter of a company’s reported half-year operating profit arriving from a rebate accrual it no longer expects to pay is not a trading result. It is a balance sheet correction that happens to land in the income statement, and the company said so itself by building a new reporting measure around it.

According to Axis Intelligence Research, net sales compounded at 21.7% annually between 2021 and 2025, but the shape of that curve changed decisively in the final year: growth fell from 25.0% in 2024 to 6.4% in 2025 as reported, gross margin gave up 3.7 percentage points, and headcount fell 10.1% after the company-wide restructuring announced in Q3 2025. Return on invested capital dropped from 63.9% to 39.3% across the same twelve months.

What Is Novo Nordisk’s Share of the GLP-1 Market?

This is the question the filings answer least directly, because neither company publishes a combined market denominator. So Axis Intelligence Research built one, using the same filings-first basis applied in our Eli Lilly statistics analysis.

The Incretin Revenue Balance (IRB)

IRB measures Novo Nordisk’s share of the incretin revenue pool it shares with its only comparable competitor, computed from both companies’ own reported product-level revenue rather than from third-party prescription panels.

Formula:

IRB = Novo incretin revenue / (Novo incretin revenue + Lilly incretin revenue)

Novo incretin revenue = Total Obesity care + Total GLP-1 diabetes, converted to USD at Novo Nordisk’s disclosed average period exchange rate Lilly incretin revenue = Mounjaro + Zepbound, as reported in USD

Snapshot date: June 30, 2026.

According to Axis Intelligence Research, the two-company incretin pool grew 43.0% year over year, from USD 31.8 billion to USD 45.4 billion. Novo Nordisk’s contribution to that growth was USD 705 million. Eli Lilly’s was USD 12,959 million. Lilly therefore took 94.8% of the category’s expansion in a period when the category expanded faster than at any point since Wegovy launched.

That is the finding worth carrying away from this page. Novo Nordisk is not shrinking. It is standing still inside a market that added USD 13.7 billion of annualised half-year revenue and handed almost all of it to one competitor.

Mia Scarlett: The instinct is to read a share loss as a demand problem. The filings say otherwise. Novo Nordisk’s obesity volumes grew – patients reached with obesity products rose 69% to 4.9 million, and Wegovy injectable volumes rose in both operating units. What fell was realised price, in every geography, in both franchises. Volume up, revenue flat means the unit economics moved against the incumbent while the incumbent was defending an installed base. Lilly was pricing into growth. Novo was repricing an existing book. Those are different exercises and they produce the divergence in the table above.

What the IRB Does Not Capture

Two firms are not the whole market. The IRB excludes tirzepatide revenue booked outside the Mounjaro and Zepbound brands, all non-incretin obesity therapy, and the compounded semaglutide channel, which is unpriced in either filing. Currency conversion uses Novo Nordisk’s disclosed average rates rather than spot, so the series measures commercial position rather than translated shareholder outcome. Axis Intelligence Research publishes the metric on a filings-only basis precisely because prescription-panel share and revenue share have diverged, and revenue is the figure both boards are managed against.

How Much of Novo Nordisk’s Revenue Comes From Semaglutide?

The Semaglutide Dependence Ratio (SDR)

SDR is the share of total company revenue generated by products containing the semaglutide molecule.

Formula:

SDR = (Wegovy injectable + Wegovy pill + Ozempic + Ozempic pill/Rybelsus) / Total sales

SDR75.5%72.8%

Source: Novo Nordisk interim filings; Axis Intelligence Research calculation. Snapshot date: June 30, 2026.

The SDR rose 2.7 percentage points while semaglutide revenue itself fell slightly, from DKK 112,756 million to DKK 112,193 million. Concentration increased because everything else shrank faster. Insulin fell 7% at constant rates globally and 17% in the United States. Victoza fell 71%. Saxenda fell 54%.

According to Axis Intelligence Research, this is the single most consequential number on Novo Nordisk’s balance of risk: three-quarters of revenue depends on one molecule whose compound patent has already expired in certain International Operations markets, whose US list price the company has committed to cut to USD 675 from January 1, 2027, and which is the subject of a revocation action filed by Sandoz at the Unified Patent Court in Milan on June 30, 2026 against the unitary patent covering the 1 mg diabetes dose across 18 EU countries.

Mia Scarlett: A concentration ratio is only alarming when the concentrated asset is losing protection, and this one is. Canada has already seen semaglutide lose exclusivity, and Novo Nordisk’s own filing records the result plainly: lower realised prices with volumes broadly stable. That is the template. The question for the next four filings is not whether the SDR falls but whether it falls because the pipeline delivers or because semaglutide erodes.

How Much Does Wegovy Make?

Total Obesity care sales reached DKK 44,064 million in H1 2026, up 19% at constant exchange rates and now 29.7% of adjusted company revenue, against 25.0% a year earlier.

The geographic split is where the franchise splits in two. US obesity care grew 6% at constant rates to DKK 24,576 million. International obesity care grew 40% to DKK 19,488 million, with EUCAN up 57%. Wegovy injectable sales in the United States fell 17% at constant rates on lower realised prices even as volumes rose, while the same product grew 53% internationally.

Wegovy Pill: The Launch, and What It Earns Per Prescription

Wegovy pill launched in the United States on January 5, 2026 and reached more than 265,000 total weekly prescriptions by the week ending July 17, 2026, against roughly 310,000 for Wegovy injectable in the same week. Cumulative prescriptions passed 5 million. It is available through more than 70,000 pharmacies and nine telehealth organisations, and self-pay prices run from USD 149 to USD 299 per month by dose.

Axis Intelligence Research computes the Oral Realised Price (ORP) to establish what the company actually books per script, as distinct from what the patient is quoted:

ORP = Wegovy pill quarterly sales / Wegovy pill quarterly total prescriptions, converted at the disclosed average period rate

DKK 3,218 million / 2.9 million prescriptions = DKK 1,110, or approximately USD 173 per prescription

According to Axis Intelligence Research, the estimated realised revenue per Wegovy pill prescription in Q2 2026 was approximately USD 173, sitting inside the disclosed USD 149 to USD 299 self-pay band rather than above it. This is an estimate: it assumes the quarter’s prescription count and its revenue cover the same scripts, and Novo Nordisk states that external data providers have not yet fully captured Wegovy pill prescriptions across all channels, which would push the true figure lower.

The equivalent calculation for Wegovy injectable is not published here. Novo Nordisk discloses injectable prescriptions as a weekly snapshot rather than a quarterly total, and annualising a single mid-July week into a quarter that ended June 30 would produce a number that looks precise and means nothing. The two products are therefore not directly comparable on this measure, and Axis Intelligence Research declines to make them look as though they are.

How Much Does Ozempic Make in 2026?

Ozempic remains the largest single product in the portfolio at DKK 59,200 million in adjusted H1 2026 sales, down 2% at constant rates from DKK 64,520 million a year earlier. The regional divergence repeats the Wegovy pattern: US Ozempic sales fell 6% at constant rates on lower realised prices in the commercial channel and lower volumes, while International Operations grew 8%, driven by EUCAN at 28%.

For full-year 2025, Novo Nordisk’s global diabetes value market share was 30.1%, down 3.6 percentage points over twelve months, against a stated aspiration of exceeding one-third. Its GLP-1 diabetes value share was 45.8% in November 2025 data, having been 51.9% in May 2025. Both figures are IQVIA measurements reported inside Novo Nordisk filings rather than independent Axis observations.

– Region China9,7440%Novo Nordisk H1 2026 filing

According to Axis Intelligence Research, H1 2026 is the first half-year in which International Operations sales came within DKK 4 billion of US Operations sales on an adjusted basis, at DKK 72,297 million against DKK 76,254 million. EUCAN alone contributed 259% of the share of adjusted growth, per the company’s own attribution table, because US Operations subtracted from it.

Mia Scarlett: EUCAN growing 20% while the US declines 4% is not a marketing outcome. It is a reimbursement outcome. European obesity access expanded through 2026 while US realised prices absorbed the Most Favoured Nations agreement, reduced Medicaid obesity coverage, and a self-pay channel where the company competes against its own list price. The segment that actually moved this half is Europe, and it moved because the payer moved.

How Many Patients Does Novo Nordisk Treat?

Novo Nordisk reached 46.5 million patients with obesity and diabetes products on a moving annual total basis at the end of June 2026, an increase of 0.8 million year over year. The composition changed far more than the total.

Diabetes care products41.6m42.8m-3%

Source: Novo Nordisk H1 2026 financial report.

According to Axis Intelligence Research, Novo Nordisk generated approximately DKK 8,993 of H1 2026 revenue per obesity patient against DKK 2,292 per diabetes patient – a ratio of 3.9 to 1. That ratio explains the entire strategic posture: 10.5% of patients are producing 29.7% of adjusted revenue, and the diabetes base that funded the company for a century is now shrinking in headcount, largely through declining human insulin volumes.

The addressable population remains far larger than the treated one. The National Center for Health Statistics measured US adult obesity prevalence at 40.3% during August 2021 to August 2023, with severe obesity at 9.4%, and its age-adjusted severe obesity figure rose from 7.7% to 9.7% across the preceding decade even as overall obesity prevalence held flat. NCHS notes that adult obesity remains above the Healthy People 2030 objective target of 36.0%.

What Is Novo Nordisk Spending on R&D and Manufacturing?

Adjusted research and development costs were DKK 21,743 million in H1 2026, alongside DKK 27,100 million of commercial investment and DKK 23,986 million of capital expenditure on property, plant and equipment. Full-year 2025 R&D was DKK 52,039 million, or 16.8% of sales, and the company added DKK 29,973 million of intangible assets that year, mainly through the acquisition of Akero Therapeutics.

The Q2 2026 income statement carries DKK 6,300 million of non-cash impairment charges on intangible pipeline assets, of which DKK 4,000 million relates to monlunabant, whose development Novo Nordisk terminated. In July the ZEUS phase 3 cardiovascular outcomes trial with ziltivekimab missed its primary endpoint, and the company has flagged that this may produce a further non-cash impairment in Q3 2026.

Set against that, the obesity pipeline advanced: the EMA approved both Wegovy pill and the Wegovy 7.2 mg single-dose pen in July 2026, on OASIS 4 data showing 16.6% mean weight loss and STEP UP data showing 20.7% respectively. In the head-to-head REIMAGINE 4 trial, CagriSema 2.4 mg/2.4 mg met non-inferiority against tirzepatide 15 mg on weight reduction but not on HbA1c reduction.

Mia Scarlett: Two impairments and a failed cardiovascular outcomes trial in a single quarter, against two European approvals and a non-inferiority result that missed one of its two endpoints. Read the R&D line the way a credit analyst reads it and the number that matters is not the DKK 21,743 million spent but the DKK 6,300 million written off, because impairment is the market’s retrospective verdict on capital already deployed. The pipeline is producing approvals. It has not yet produced a second molecule.

What Is Novo Nordisk’s 2026 Guidance?

On August 4, 2026, Novo Nordisk raised full-year guidance for the second consecutive quarter.

Guidance measureAugust 4, 2026May 6, 2026February 3, 2026

Guidance has been raised twice and still describes a year of contraction at the midpoint. According to Axis Intelligence Research, the guidance floor has moved 7 percentage points since February while the ceiling has moved 5, which narrows the range rather than shifting the outlook to growth – the company is signalling improved confidence in a decline, not a return to expansion.

The capital return continued regardless: DKK 41,200 million went back to shareholders in H1 2026 through DKK 35,300 million of dividends and DKK 5,900 million of buybacks, and the board declared an interim dividend of DKK 3.75 per share paid in August 2026. Net debt stood at DKK 86,523 million at June 30, against total equity of DKK 221,277 million.

Methodology

Collection. Every figure on this page was fetched from a primary document during production on August 20, 2026, and logged with its URL and retrieval date in the accompanying CSV. The core sources are Novo Nordisk’s financial report for January to June 2026 (Company Announcement No 48/2026, furnished to the SEC on Form 6-K, August 4, 2026), Novo Nordisk’s financial report for January to June 2025 (Form 6-K, August 6, 2025), the Novo Nordisk Annual Report 2025, Eli Lilly’s second-quarter 2026 results (Form 8-K, August 5, 2026), and NCHS Data Brief No. 508. No figure on this page was taken from a secondary aggregator, and no figure was reconstructed from memory.

Currency. Novo Nordisk reports in Danish kroner. Where this page converts to US dollars, it uses the company’s own disclosed average period rates – 6.40 DKK per USD for H1 2026 and 6.85 for H1 2025 – rather than spot rates, so that revenue comparisons reflect the same translation basis the company used to report the underlying figures.

Adjusted versus reported. Novo Nordisk introduced adjusted sales and adjusted operating profit in 2026 to exclude the 340B provision reversals, major impairments and major legal matters. This page uses adjusted figures for operating comparisons and states reported figures wherever the difference is material, because the two answer different questions and quoting either alone would mislead.

Formulas. IRB, SDR and ORP are Axis Intelligence Research constructions. Their components, weights and calculation steps are disclosed inline above, and every input carries a CSV row with its own source. A reader with the two filings can reproduce all three readings.

Comparability notes. The H1 2025 semaglutide figure uses reported sales, since no 340B adjustment applied to Novo Nordisk’s Q1 2025 or to the equivalent product lines in the same way; the H1 2026 figure uses adjusted sales. Novo Nordisk restated its free cash flow definition effective Q1 2026 and restated comparatives accordingly. Market share percentages sourced to IQVIA are third-party measurements reported inside company filings, not Axis observations, and are labelled as such throughout.

An unreconciled discrepancy. Novo Nordisk’s H1 2026 financial report describes the company as employing about 66,700 people. Company announcements filed in the same week state about 68,800. The audited year-end 2025 headcount in the Annual Report 2025 is 69,505. Axis Intelligence Research publishes all three rather than selecting one, and treats the audited annual figure as canonical for year-on-year comparison.

Frequently Asked Questions

Why do Novo Nordisk’s reported and adjusted sales differ by DKK 26.8 billion in H1 2026?

Novo Nordisk had accrued a provision against statutory discounts under the US 340B Drug Pricing Program, which requires manufacturers to offer discounted outpatient drugs to safety-net providers. In Q1 2026 it reversed USD 4.2 billion of that provision, equal to DKK 26,760 million, and the reversal flows through the sales line. It has no cash impact, so adjusted sales exclude it. Reported H1 2026 sales of DKK 175,311 million are the IFRS figure; DKK 148,551 million is the underlying trading figure.

Has Eli Lilly overtaken Novo Nordisk in incretin revenue?

Yes, on a combined-revenue basis. Axis Intelligence Research’s Incretin Revenue Balance puts Novo Nordisk at 39.1% of the two-company pool for H1 2026 against 53.6% for H1 2025, with Lilly’s Mounjaro and Zepbound generating USD 27,693 million against Novo Nordisk’s USD 17,747 million of obesity care and GLP-1 diabetes revenue. Prescription-volume share and revenue share are not the same measure, and Novo Nordisk continues to report leadership in branded obesity new patient starts in the US. Product-level detail on the competing portfolio sits in our Mounjaro and Zepbound revenue breakdown.

What happens to Novo Nordisk’s US revenue when the 2027 list price cut takes effect?

Novo Nordisk has committed to lowering the wholesale acquisition cost of Wegovy and Ozempic to USD 675 per month from January 1, 2027, reductions of approximately 50% and 35% respectively. The company states the change will affect its cash flow in 2027. List price is not realised price, since rebates already discount the gross figure substantially, so the net revenue effect depends on how much of the existing gross-to-net spread the cut absorbs – a figure Novo Nordisk does not disclose at product level.

How exposed is Novo Nordisk to semaglutide patent expiry?

Semaglutide products generated 75.5% of adjusted H1 2026 sales. The compound patent has expired in certain International Operations markets, and Canada has already seen loss of exclusivity, which Novo Nordisk reports produced lower realised prices with broadly stable volumes. On June 30, 2026, Sandoz filed at the Unified Patent Court in Milan seeking revocation of the unitary patent covering the 1 mg semaglutide diabetes dose across 18 EU countries. Cipla has filed an ANDA for a generic Ozempic in the US, and Novo Nordisk has sued for infringement in the District of New Jersey.

Is the Wegovy pill cannibalising Wegovy injectable sales?

The filings show both running concurrently rather than one displacing the other. In the week ending July 17, 2026, injectable Wegovy ran at roughly 310,000 weekly US prescriptions and the pill at more than 265,000, with the pill’s uptake described as driven by GLP-1-naive patients. US Wegovy injectable revenue fell 17% at constant rates in H1 2026, but Novo Nordisk attributes that to lower realised prices with volumes rising, not to volume loss. Combined Wegovy product sales in the US grew 7% at constant rates.

What is the Medicare GLP-1 Bridge programme and does it show up in these numbers?

Bridge is the pilot that extends Medicare Part D coverage of obesity medicines under the Most Favoured Nations agreement Novo Nordisk reached with the US administration in November 2025. It took effect on July 1, 2026 – after the close of the reporting period covered by this page – so it contributes nothing to H1 2026 revenue. Novo Nordisk describes initial uptake as encouraging, and the effect will first appear in the nine-month report due November 4, 2026.

Why is Novo Nordisk’s diabetes patient count falling while obesity patients rise?

Patients reached with diabetes products fell from 42.8 million to 41.6 million year over year, a decline Novo Nordisk attributes mainly to fewer patients treated with human insulin. Human insulin sales fell 21% at constant rates in H1 2026 and vulnerable patients reached through affordability programmes and tenders fell 12%, largely on lower tender volumes in Emerging Markets. Obesity patients rose 69% to 4.9 million over the same period.

Which region is actually driving Novo Nordisk’s growth in 2026?

EUCAN. Adjusted EUCAN sales grew 20% at constant rates to DKK 37,124 million in H1 2026, with obesity care in the region up 57%, while US Operations declined 4% and Emerging Markets declined 7%.
